Amenorrhea

Definition: Amenorrhea simply means the absence of menses or cessation of menses. It happens when a woman of reproductive age misses her monthly period consecutively for up to three months. It can also happen if a girl has not seen her period for the first time up to the age of 16 years.

Classification/Types:

(A) Primary Amenorrhea (B) Secondary Amenorrhea

  • Primary Amenorrhea results when a woman has not seen her menses for the first time in her life up to 16 years, even though she has reached the age to menstruate.
  • Secondary Amenorrhea results when a woman who has been seeing her period suddenly or gradually stops seeing her period for whatever reason. Whether primary or secondary, Amenorrhea gives a lot of emotional trauma to a woman, except when desired during pregnancy.

Causes of primary Amenorrhea:

  • Family history of amenorrhea or early menopause
  • Genetic or chromosomal abnormalities, e.g., Turner’s Syndrome

Diagnosis of Amenorrhea:

Tests required for amenorrhea:

Tests required in amenorrhea depend on the suspected cause and to some extent will be determined by the doctor managing the case. These tests may include:

  • Pregnancy test, especially in secondary amenorrhea, and the woman is sexually active.
  • Hormonal profile if PT is negative.
  • Transvaginal ultrasound scan if ovarian cyst or PCOS is suspected.
  • MRI if Pituitary origin is suspected.
  • Karyotyping and genetic studies if chromosomal abnormalities are suspected.

Treatment for amenorrhea:

The need for treatment depends on the cause. Always visit a doctor if you have amenorrhea.

  • If due to pregnancy, no treatment is required as periods return to normal after childbirth and breastfeeding.
  • For primary Amenorrhea when a girl is up to 16 years, treatment will be initiated after finding out the cause.
  • For menopause, no treatment may be required.
